Scheduled registration for the 2004/2005 season is now complete. A late fee of $20 per family is now in effect. Late registrations are being accepted in one of two ways.
1. Online registration may be used at http://www.sportability.com/dusl. DUSL is introducing online registration starting April 1, 2004 for late registration! Online registration with sPortability is easy, secure, and hassle-free. The entire process is encrypted, so your information is secure. Most registrations take about 10 minutes from start to finish, and you will receive a confirmation email confirming your registration. Payment is made with credit cards only.
2. Late registration packets are available at 11795 Shadow Drive in a blue box by the front door. Completed packets can be returned to the blue box.
Children who were born between August 1, 1985 and July 31, 2000 are eligible to register. Please see this chart for more detail.

Work Deposit: $50.00 per family. If you volunteer three hours for the season, the $50 work deposit is carried over to the next season or you can request a refund in writing after the end of the season.
Uniforms: Uniforms are not included in the registration fee. Contact Tom Rhoads at equip@dusl.org to order a uniform or separate pieces. For the recreational division the U12-U19 uniform consists of two jerseys, one green and one white, black shorts and white socks. The U6 -U10 player uniforms are a white & green reversible jersey, black shorts and white socks.
Materials Required: Proof of age is required to register. Proof of age must be a copy of the child's birth certificate, passport, alien registration card or DMV issued forms of identification (drivers license or CA state ID). Hospital or Baptismal certificates will NOT be accepted.
Late Fee: $20 late fee per family applies as of 3/31/04.
Refund Request Fee: A $25 admin fee will be assessed on requests for refunds after July 1st.
Please Note:At the DUSL regularly scheduled meeting held on January 14, 1999, the Board voted to deny any special requests for placement on teams with the exception of siblings in the same age bracket. Accordingly, beginning with 1999 registration, no requests to be placed on a specific coach's team or to be placed on the same team with certain children will be considered. Please direct any questions about this policy to any of the Board members by calling the hotline or emailing the Board.
